T-cell receptor therapy modifies a patient's T cells so they can recognize a specific cancer-related antigen. Unlike CAR T-cell therapy, TCR therapies recognize targets presented through HLA molecules, so eligibility can depend on both the tumor marker and the patient's HLA type.

The FDA has approved afamitresgene autoleucel for certain adults with unresectable or metastatic synovial sarcoma who meet specific MAGE-A4 and HLA criteria after chemotherapy. Most other TCR approaches remain investigational. Testing, manufacturing, prior treatment, and center experience determine access.
Procedure and practical considerations
The treatment requires cell collection, laboratory manufacturing, lymphodepleting chemotherapy, and close monitoring. Serious risks can include cytokine release syndrome, low blood counts, infection, and organ complications. Manufacturing delays or failure and rapid cancer progression while waiting are practical concerns to discuss.
